Emergencies happen at any time, any place, and at any
age. Are you prepared? Can you name all of the medications
and dosages you take, including over-the-counter drugs?
When was your last tetanus shot? Can you list all of
your surgeries and the dates? These questions are frequently
asked at medical appointments. Accurate answers are
of utmost importance because doctors base their diagnoses
on what they are told. Many patients rely on their memory,
forgetting important facts, especially in emergency
situations.
People are urged to take responsibility for their health
care. Doctors and medical organizations want their patients
to record their medications, hospitalizations, and surgeries;
however, you cannot go to a bookstore and find a specific
organizer to record your medical information.
My Medical Assistant® standard edition is a full-size book.
There is a place in the organizer to record most of
the facts people are asked on a medical questionnaire.
The book consists of 128 refillable pages in a 9”
x 6” loose-leaf binder. There are 35 forms, including
a medication list, medication schedule, hospitalizations,
doctor’s notes, family medical history, dental
